Skilled Nursing

The goal of skilled nursing visits is to educate, monitor and promote independence with care. Nursing care is provided by Registered Nurses (RNs), and Licensed Practical Nurses (LPNs). If you get services from an LPN, your care will be supervised by an RN. The nurse is your health care team leader and coordinates the services you receive. The nurse also communicates frequently with your physician to update your plan of treatment.

Our registered nurses and licensed nurses can provide:

  • Disease education and management

  • Administer and manage medications

  • Patient and family education

  • Pain and symptom management

  • Wound care management

  • Intravenous Therapy

Wound Care

A wound care specialist is a healthcare provider who focuses on evaluating and treating non-healing wounds. Our provider can help patients determine the best treatment for wound care. A wound care specialist is consulted when conventional treatments such as dressings and ointments are not enough to heal a wound, they have access to the most up-to-date wound care technology and the training to use it. You may see a wound care specialist in addition to your primary care physician or home healthcare provider until your wound is healed. Wound care specialists are covered by most insurance plans and often do not require a referral.

Physical Therapy

Physical therapy staff works to restore and promote optimal strength and physical function to patients who have limitations or disabilities as a result of disease or injuries. Working with our physical therapists helps patients restore, maintain and promote optimal physical function, wellness, and quality of life. We work closely with each physician to create an individualized treatment plan that incorporates advanced rehabilitation techniques and therapeutic modalities. This is to help patients regain or improve their physical abilities.

Physical Therapists can:

• Restore function and strength
• Improve balance and ability to walk
• Home safety and equipment needs

Occupational Therapy

Occupational therapy staff works with patients to adapt the way they bathe, dress, groom, perform homemaking tasks and prepare meals in spite of physical limitations. We work closely with each physician to create an individualized treatment plan that meets the patient’s goals. Occupational Therapists assess the patient’s abilities, tailor programs to restore physical functions, and devise ways to help those with permanent limitations to carry out everyday functions.

Occupational Therapist can:

• Improve self-care and daily tasks

• Home safety and modification

• Fine motor coordination therapy

Speech Therapy

Speech Language Pathology services are a rehabilitative service that may be utilized for patients with speech pathology problems such as speaking or swallowing difficulties. Speech Therapists work to restore functional speech and cognition, improve articulation and language, treatment of swallowing disorders.

Dietician

Our dietician services educate the patient on a prescribed diet and weight management plan. They optimize their health condition through good nutrition.

Medical Social Work

Social workers evaluate the social and emotional factors affecting patients and provide counseling to the patient and the family unit. This might include counseling or help in finding resources within your community. The Medical Social Worker completes an assessment and evaluation of psychosocial, emotional, and financial needs. Their services improve patients’ support system, health factors, community resources, etc.
